**POSITION TITLE:** Advanced Practice Clinician (NP/CNM/PA) **LOCATION**: Remote Care Team (Tuesdays), White River Junction ( Wednesdays), Barre (Thursdays), Rutland ( Fridays) **HOURS:**Part Time, 28.5/week , Hourly (Non-Exempt) **UNION MEMBERSHIP:**This position is represented by AFT union in VT **POSITION PURPOSE:****Advanced Practice Clinicians (APC)** provide patient-centered, personalized health care to clients.
