Job Description: Family Medicine APP Full-time-Float both FM/IM Clinic Locations Float Salt Lake Valley- Intermountain Health Covering both Family and Internal Medicine Clinics in the Salt Lake Valley, 25 Buildings, covering long term leaves, and current provider templates. Minimum Qualifications: Current state license as a Physician Assistant or Advance Practice Registered Nurse (APRN). Certification as a Physician Assistant or Nurse Practitioner (Credentialing requirement). Eligible for State Controlled Substance License II through V. Eligible for Federal DEA Certificate II through V. BLS Certification for Healthcare Providers. Other certification as required (e.g. ACLS, PALS, TNCC, NRP). Preferred Qualifications: Two (2) or more years of experience preferred. Master’s degree. Education must be obtained from an accredited institution. Degree will be verified.
