Case Manager (Care Coordinator) Full-Time | Non-Exempt Make a Difference Every Day The Source is seeking a compassionate, organized, and mission-driven Case Manager to walk alongside women and families by providing care coordination, support, and access to life-changing resources. What You'll Do As a Case Manager, you will: Conduct client needs assessments and develop individualized Care Plans Connect clients with community resources and government assistance programs Provide ongoing encouragement, coaching, and care coordination Conduct follow-up calls, home and hospital visits Maintain accurate client documentation using Salesforce Lead and support client groups while collaborating with clinic staff to ensure excellent client care What We're Looking For Required Qualifications Committed Christian with a personal relationship with Jesus Christ Passion for serving women, children, and families while upholding the sanctity of human life Bachelor's degree in a related social services field, Registered Nurse (RN), or Certified Community Health Worker Strong organizational, communication, and computer skills Ability to manage confidential information with professionalism Preferred Qualifications Case management or care coordination experience Experience in coaching, mentoring, or motivational interviewing Master's Degree in Social Work (Texas Licensed) Why Join The Source?
