We are looking for an empathetic, organized, and driven master's Level Case Manager. In this role, you will assess client needs, design individualized care plans, and connect individuals with critical community resources. You will advocate for clients and help them navigate complex support systems. This position will work with adult/gero acute patients.Key ResponsibilitiesConduct detailed interviews to identify the psychological, medical, and social needs of each client.Develop specific, step-by-step care plans to help clients reach their goals.Provide individual, group, and family psychoeducation-oriented care using various professional treatment modalities.Coordinate individualized discharge planning for patients with physician.
