Concordia Visiting Nurses is looking for a dedicated Clinical Scheduler to join our busy home health office located in Cabot, PA. As a Clinical Scheduler you will collaborate with our clinical field staff to coordinate home care visits and support the daily scheduling operations of our agency. The Clinical Scheduler needs to be: • Proficiency in computer applications, particularly Word and Excel. • Strong organizational skills with a keen attention to detail. • Effective communication and interpersonal abilities. • Capability to prioritize and manage daily tasks. • High school diploma or equivalent required. What you will do as a Clinical Scheduler: • Develop new daily and weekly schedules for staff and create annual model calendars. • Verify clinician-provided schedules against computer-generated schedules and make necessary adjustments. • Ensure recent visits are accurately recorded to avoid overlooking patient recertifications. • Confirm that clinician-written visit frequencies align with actual visit patterns, contacting clinicians for corrections as needed. • Reassign patients to covering clinicians during vacations and other absences. • Process orders and assign them to the appropriate clinician, addressing any computer-related issues. • Schedule Telehealth installations and removals. • Respond to general inquiries from staff and patients via phone, email, or in person. • Identify and report any issues or concerns regarding patients and staff. • Communicate any changes in the daily schedule to relevant supervisors or staff members.
