Certified Nursing Assistant (CNA) – Home Health Prime Home Health | San Antonio, TX Make a difference where it matters most—at home. Prime Home Health is seeking compassionate, dependable Certified Nursing Assistants (CNAs) to provide exceptional in-home care to patients throughout the San Antonio area on a PRN basis. Help patients maintain their independence while delivering high-quality, one-on-one care in the comfort of their homes. Responsibilities Assist with bathing, dressing, grooming, toileting, and mobility Monitor and document vital signs Provide medication reminders and assistance Prepare light meals and encourage proper nutrition Perform light housekeeping and errands Offer companionship and emotional support Document patient care accurately and timely in our EHR system Communicate changes in patient condition to the supervising nurse Qualifications Current Texas CNA Certification Current CPR certification TB test taken within the past year High school diploma or equivalent Computer skills required (EHR experience preferred; Kinnser/WellSky, Slack a plus) 2 Years Home Health experience preferred Reliable transportation and valid driver's license Strong communication skills and a compassionate, patient-focused attitude Why Join Prime Home Health?
