Job Title: Floor Nurse (LPN or RN) Reports To: Director of Nursing/Executive Director Department: Nursing Location: On-site, Lynchburg, VA Employment Type: Full-time (LPN ONLY) | Part-time (LPN ONLY) | PRN (LPN or RN) | Hourly | Non-Exempt Compensation : The pay range for this position is $30 - $45 per hour, set in good faith based on the responsibilities of the role, qualifications, and market conditions. Applicable shift differentials: $1 per hour 4:00-7:00pm $2 per hour 7:00pm-7:00am Job Summary Mainspring Recovery is seeking a reliable, detail-oriented Floor Nurse (LPN or RN) to provide medication administration and routine nursing support for residential clients. The primary responsibility of this role is safe, accurate, and timely medication administration while supporting basic nursing assessments and coordination of care as directed by medical and psychiatric providers. The Floor Nurse works collaboratively with the interdisciplinary treatment team to ensure continuity of care, client safety, and compliance with nursing standards, organizational policies, and regulatory requirements. Key Responsibilities Administer prescribed medications accurately and safely in accordance with physician orders, medication administration records (MAR), and organizational policy. Prepare, organize, and maintain medication carts, ensuring medications are stored securely and arranged alphabetically by client name. Review, count, and document controlled substances at the beginning and end of each shift in accordance with policy. Monitor clients for medication effectiveness, side effects, and adverse reactions, and report concerns to the appropriate provider. Perform routine nursing tasks including vital signs, weight checks, blood sugar monitoring, pain assessments, and other clinical observations as ordered. Complete intake-related nursing tasks for new admissions as assigned, including vital signs, weight, and medication review. Review and update medication lists daily to reflect changes in prescriptions, dosages, or administration times. Document all nursing interactions, medication administration, and clinical observations accurately and timely in the electronic medical record (EMR). Verify laboratory work is completed as ordered, review results, and notify medical or psychiatric providers of abnormal values. Participate in daily sick call and assess client concerns to determine appropriate follow-up or referral. Coordinate with providers, admiissions staff, and clinical leadership regarding clients with complex medical needs or polypharmacy concerns. Consult with counselors, therapists, and recovery support staff regarding client medical status and ongoing care needs. Support scheduling of outside medical appointments as directed by providers. Maintain compliance with infection control, safety standards, and nursing scope of practice. Perform all other duties as assigned to support nursing operations and continuity of care. Minimum Qualifications Education / Credentials Current Virginia state Licensed Practical Nurse (LPN) or Registered Nurse (RN) license required. Must maintain active licensure in accordance with the standards of the Commonwealth of Virginia. Experience Minimum of six (6) months of clinical experience as an LPN or RN. Experience in behavioral health or residential substance use disorder treatment preferred. Knowledge / Skills Strong knowledge of medication administration and nursing documentation standards. Excellent verbal and written communication skills. Ability to visually assess client behavior and identify changes in physical or emotional condition. Strong organizational and time management skills. Ability to work independently while collaborating effectively with a multidisciplinary team. Working Conditions On-site, shift-based direct care position. Full-time, part-time, or PRN schedules available. Evenings, weekends, and holidays required based on staffing needs.
