Franciscan Health Lafayette Central 1501 Hartford St Lafayette, Indiana 47904 Once a Physical Therapist has evaluated a patient and determined the course of action, the Physical Therapist Assistant of Franciscan often takes charge. This job requires someone who is comfortable following and giving orders, and someone with a careful eye who can modify and ensure correct patient performance. WHAT YOU CAN EXPECT Report patient progress and collaborate with evaluating therapist to implement therapy plan of care. Perform comprehensive physical therapy treatment and related follow-up services, including discharge planning. Document and submit patient reports within the established timeframe. Collaborate with patient and other healthcare personnel or outside agencies regarding coordination of care. Exercise clinical judgment following an occupational therapy plan of care. Train and educate patients and family members or other support, and documents understanding of treatment plan and goals.
