Our mission to become the 'Employer of Choice' has led up to wining a 'Top Workplace' Award by the Indy Star for the past 5 years, and we are not finished yet! ### Job Description - Full-time Hospice RN Case Manager position - Schedule: Monday thru Friday, 8:00 am -5:00 pm - Available territory openings: Case Manager typically assigned to cover 1-2 counties near the Office. - Performs physical and psychosocial assessment on all hospice patients. - Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. - Responds to the physical, mental, social and emotional needs of terminally ill patients and their families. - Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary. - Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate. - Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc ### Benefits - Competitive salary - Mileage reimbursement - Excellent PTO program (up to 136 hours in first year) - Every Friday is payday! - Health, dental, vision - 401k match program - Great team environment! ### Requirements - One year Hospice experience preferred - 2 years of RN experience required - Licensed Registered Nurse in State of Indiana - Current driver's license - Must have reliable transportation and insurance
