Job description

The Hospital Access Liaison provides rapid, compassionate support to individuals recently discharged from the Emergency Department or inpatient units. This role focuses on linking high-risk clients to community and behavioral health services, supporting continuity of care, and promoting recovery and wellness. **Full-time, benefit eligible position.
