City/State Norfolk, VA Work Shift Weekend Days Overview: Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/ Registered Nurse (RN)- Full Time Weekend Dayshift ! This position is eligible for up to a $20,000 sign-on bonus for qualified candidates. Status: Full-Time, permanent position (36 hours OR 40 hours) Standard working hours: 3x12 weekend schedule/ 36 hours: 8:00 AM to 8:30 PM EST, Friday, Saturday, and Sunday OR Saturday, Sunday, and Monday, including a 30-minute lunch break 4x10 schedule/ 40 hours: 8:00 AM to 6:30 PM EST, Friday through Monday, including a 30-minute lunch break Eligible for weekend shift differential The position requires training Monday through Friday from 8:00 AM – 4:30 PM for the first several weeks before transitioning to the full-time weekend day shift. Location: This is an onsite position at Sentara Norfolk General Hospital, 600 Gresham Drive, Norfolk, VA 23507. Job responsibilities: The Inpatient Case Manager RN is responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS. Directly support those in hospital patients identified in need of comprehensive case management. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver. Functions in one of the following practice settings: Hospitals Only Education: Bachelor's Degree in Nursing preferred Certification/Licensure Registered Nurse (RN) in VA or NC or Compact/Multi-state REQUIRED BLS within 90 days of hire. Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.
