Job Description Licensed Practical Nurse (LPN) Symbii Home Health and Hospice Symbii Home Health and Hospice is seeking a Licensed Practical Nurse to join our team! Flexible hours: As a home health and hospice licensed practical nurse, you make your own schedule based upon your needs and the needs of your patients. You can run an errand, pick up your kids, or go to an appointment between patients. Minimal Weekends: A nurse at Symbii works an average of 1 weekend every 1-2 months and does minimal visits during their on-call weekend. Consistent Hours: We are growing! What will you do as an LPN at Symbii? Provide direct patient care as defined in State’s Nurse Practice Act. Implement plan of care initiated by the registered nurse. Provide accurate and timely documentation consistent with the plan of care. Collect assessment data and perform observation to report to and collaborate with RN(s) and physician(s). Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse and/or Assistant Director of Nursing or Director of Nursing.
