Medical Director, Practice Performance

Job description

This is not a traditional Medical Director role. Your mandate is simple: **turn data into action, and action into better care, better outcomes and more efficient care.** You’ll partner with clinicians on the ground, using data, coaching, and technology (**Clover Assistant**) to influence how care is delivered—day to day, patient by patient. **What you'll do:** - Partner directly with network physicians and care teams to improve quality, outcomes, and total cost of care - Encourage network primary care physicians to refer patients to in-home care and other Clover Care Services when clinically appropriate, connecting patients with the care that best supports their needs and outcomes - Identify performance gaps and translate data into clear, actionable clinical changes - Coach providers (1:1 and group) on evidence-based care, documentation, and population health strategies - Drive adoption and effective use of Clover Assistant in real clinical workflows - Be the tip of the spear for implementation of clinical programs with partner providers - Build trusted relationships with clinicians—serving as a peer advisor, not just a directive voice - Collaborate routinely with fellow Medical Directors, account managers, Data Science, Operations, Analytics, and Provider Success to translate insights into coordinated action and align clinical and business goals - Support and mentor market-level clinical leaders to drive performance across the network **What winning looks like:** - Measurable improvement in clinical quality metrics and patient outcomes - Reduction in total cost of care through better clinical decision-making - Increased provider engagement and consistent use of Clover Assistant - Providers change behavior—not just understand the data - Strong, trusted relationships across the provider network **Why this role is hard (and worth it):** - You’re influencing clinicians—without direct authority - Changing clinical behavior is complex and requires trust, credibility, and persistence - You’ll need to balance data, clinical judgment, and real-world constraints - Your impact is highly visible—and directly tied to patient outcome **Who you are:** - MD or DO with an active, unrestricted license; board certified (Primary Care preferred) - Clinically credible with 5+ years of practice experience - Experience in value-based care, population health, or risk-based models (MSO, ACO, MA) - Proven ability to influence and coach peer clinicians - Comfortable using data to drive clinical decisions and behavior change - High comfort with EMRs, clinical tools, and technology-enabled care models - Strong communicator who builds trust quickly with physicians and care teams --- **Benefits Overview**: - **Financial Well-Being**: Our commitment to attracting and retaining top talent begins with a competitive base salary and equity opportunities.
