You can learn about our benefits here: Job Summary In an assigned City agency or program, may direct, administer, oversee and coordinate the planning, development, and organization of the activities and operations of a program. Advises agency head on the status, findings and problems of projects and programs; recommends to same project policies, practices and problem resolutions. With a high degree of independence, develops, implements, recommends changes in and approves program policies and procedures; directs and participates in the interpretation of policies and procedures. May evaluate the performance of, discipline and recommend the hiring, firing and promoting of subordinate program personnel or unit supervisors. May direct the preparation of grant proposals for agency programs and initiatives May direct the preparation and administration of a program budget, budget recommendations and the monitoring of budget expenditures. May coordinate and oversee, upon referral from hospitals, clinics and other agencies, the provision of services to clients diagnosed with medical conditions or problems or needing care. May oversee the maintenance and accuracy of detailed client case records and files; ensures the confidentiality of client case records and files. May participate with others in conducting educational and informational meetings, programs and public health fairs with the general public and targeted at-risk populations. Evaluates program activities to determine program effectiveness and to ensure achievement of program objectives; reviews policies and procedures to assess effectiveness, makes recommendations for improvement and reports findings to super­iors. Speak before community groups and other organizations presenting, explaining and promoting program services. Coordinates program activities with other agencies and organizations explaining program services to and obtaining financial and other assistance from such agencies and organizations and providing assistance to them. Operates a variety of standard office machines including personnel computers and related standard software and hardware to record, edit, store and revise correspondence, reports, statistical tables, forms and other materials. Answers sensitive, controversial, unusual or complicated questions or complaints regarding work procedures or depart­mental or grant-funded programs, operations, services or functions. Compiles complex reports, forms and summaries for agency head, reviewing, interviewing and investi­gating a variety of sources to secure complete and accurate information. Oversees and coordinates studies, surveys and research projects. Essential Functions Conduct in-person and telephonic visits to evaluate care and health status of participants for the nurse monitoring program for clients receiving personal assistance services (pas), completing a minimum of 20 visits per week and a minimum of 25 billable hours per week. Assesses the quality of care provided by the residential service agency (RSA) and individual caregivers to ensure personal assistance services (PAS) follow the client’s plan of service (POS). Schedule appointments with clients or clients’ representatives. Review client documentation and complete required assessment and evaluation tools. Manage assigned caseload, review plans of service, coordinate and make community referrals. Use the long-term services and supports system (LTSS) for documentation, billing, and assessment history. Liaise with supports planners and assessment, evaluation, and review services facilitate the health and safety of participants in a variety of community settings. Complete time-sensitive outreach documentation and mileage reporting. Perform related work as required, including emergency essential duties as needed. Minimum Qualifications Education: Have a bachelor’s degree from an accredited college or university AND Experience: Four years of experience in coordinating, monitoring and processing cases involving the provision of health, social, vocational or rehabilitation services to clients, including two years of experience supervising subordinate staff or equivalent combination of education and experience. Preferred experience: experience working with older adults and/or persons with disabilities. OR Equivalency Notes: Have an equivalent combination of education and experience. Licenses, Registrations, and Certificates: Have a valid Maryland Class C Noncommercial driver's license or an equivalent driver's license and be eligible to obtain a Baltimore City driver’s permit.
