Liquid Personnel is seeking an experienced Occupational Therapist to support the delivery of adult social care through timely and effective initial assessments for its client based in the London Borough of Tower Hamlets.
This is an exciting opportunity for a skilled Occupational Therapist to join Thackley Green Specialist Care Centre, providing specialist rehabilitation and discharge planning support. The successful candidate will play a key role in helping patients transition safely and effectively from hospital settings back into the community.
Working within a multidisciplinary team, you will conduct comprehensive therapy assessments, coordinate safe and timely discharges, and support individuals in regaining independence through tailored rehabilitation programmes and the provision of suitable equipment and adaptations. This role offers the chance to make a meaningful impact on patient outcomes while contributing to high-quality, person-centred care.
The Role:
As an Occupational Therapist, you will lead therapy assessments and rehabilitation for patients admitted to the service, primarily following discharge from hospital.
You'll undertake access visits to patients' homes, recommending minor adaptations and equipment to support safe discharge, while working closely with multidisciplinary professionals to ensure seamless transitions of care.
There may also be opportunities to support the wider Discharge to Assess pathway by managing additional referrals when required.
