Liquid Personnel is currently recruiting for an experienced Occupational Therapist to join the Discharge to Assess Team at the Redbridge.
This is an excellent opportunity to join a supportive, locality-based multidisciplinary team delivering integrated community health and Adult Social Care services. You'll play a key role in supporting safe hospital discharges while promoting independence through high-quality, person-centred Occupational Therapy interventions.
The Role
As an Occupational Therapist within the Discharge to Assess Team, you'll manage a caseload of approximately four cases per week, undertaking both standard and complex assessments for adults with a wide range of health needs.
Working autonomously and alongside multidisciplinary colleagues, you'll assess for specialist equipment, manual handling solutions, and major adaptations to ensure people can return home safely and maintain their independence.
