Occupational Therapist - Reablement Team

Liquid Personnel is recruiting for an experienced Occupational Therapist to join Wiltshire Council's Reablement Team, supporting adults following hospital discharge and helping them regain independence.

This is a fantastic opportunity to work within a supportive team delivering person-centred assessments and reablement interventions across home, hospital, and community settings.

What will your responsibilities be?

• Complete Occupational Therapy assessments following hospital discharge
• Develop tailored reablement plans to maximise independence
• Provide advice on equipment, moving safely, and best practice
• Work collaboratively with colleagues to support decision-making and positive outcomes
• Carry out community visits while managing a varied caseload

Qualifications & Experience:

To be successful in this role you must have,

• Qualified Occupational Therapist
• Minimum 2 years' post-qualification experience
• Full UK driving licence
• Please note: Travel and accommodation expenses from outside Wiltshire are not covered.
