Giving Home Health Care is seeking an **Outreach Coordinator** to drive patient growth and expand our presence throughout the **Augusta, GA and Aiken, SC markets**. In this highly visible role, you will serve as a community ambassador for Giving Home Health Care, building strategic relationships with referral sources, healthcare providers, community organizations, unions, and other key stakeholders. You will play a direct role in patient acquisition and market growth by identifying opportunities, generating referrals, and guiding prospective patients through the intake process. #INDEST ### Responsibilities - Develop and execute strategic outreach and business development initiatives designed to increase patient referrals, market penetration, and revenue growth. - Build, cultivate, and maintain strong relationships with healthcare providers, unions, case managers, community organizations, and other referral sources. - Identify and pursue opportunities to expand Giving Home Health Care's presence within the Augusta and Aiken markets. - Act as a trusted resource for prospective patients and families, educating them on available services and guiding them through the intake and onboarding process. - Conduct proactive outreach to potential patients and referral partners through phone calls, in-person visits, networking events, and community engagement activities. - Maintain regular communication with patients, referral sources, and key stakeholders to ensure an exceptional experience and strong engagement. - Represent Giving Home Health Care at community events, senior expos, health fairs, patient appreciation events, and industry networking functions. - Partner with internal departments to ensure a seamless transition from referral to admission and ongoing patient support. - Monitor market trends, competitive activity, and referral patterns to identify opportunities for growth. - Track outreach activities, referral pipelines, and conversion metrics to maximize performance and return on investment. - Build and maintain relationships with hospital systems, medical offices, and records departments to facilitate patient advocacy and timely access to services. - Navigate healthcare approval processes and work collaboratively with internal teams to help patients access the care and support they need. ### ### What Success Looks Like - Consistently generates qualified patient referrals and new business opportunities. - Builds a strong network of referral partners throughout the community. - Expands Giving Home Health Care's visibility and reputation within the market. - Delivers an exceptional experience for patients, families, and referral sources. - Contributes directly to patient growth, market expansion, and overall business success. ### Qualifications - Minimum of 2-3 years of sales experience - Proven experience in community outreach, public relations, or a similar role. - Previous experience in healthcare outreach, marketing, or sales (home health experience is a plus). - Excellent communication and interpersonal skills. - Strong organizational and project management abilities. - Proficiency in Microsoft Office Suite and social media platforms. - Ability to work independently and as part of a team. - Passion for community engagement and making a positive impact. - Valid driver’s license and reliable transportation for travel within the community.
