Day - 08 Hour (United States of America) This is a Stanford Health Care job. A Brief Overview The Patient Safety Specialist is responsible for management of the incident/event reporting system, leading event review and cause analysis, educating teams and leaders on safety and high reliability, and collaborating across departments. This role also supports strategic safety initiatives within the organization. Under the direction of the Director of Patient Safety, the Patient Safety Specialist performs a range of functions that support the delivery of safe and effective patient care, the reduction of medical/health care errors, improvement of the facility-wide patient safety program, and timely reporting and follow-up corrective action is taken in response to such events/incidents in accordance with the requirements of hospital policies, the Joint Commission, and other external regulatory and accrediting bodies. Locations Stanford Health Care What you will do Patient Safety Incident Management and Event Review: Perform timely reviews of patient safety events, including medical record reviews, interviews, and cause analysis. Provide and support recommendations to identify, mitigate, and/or eliminate patient harm. Ensure timely documentation for regulatory reporting requirements. Present to forums such as the organization’s patient safety committee and other committee structures to disseminate learnings. Data Management and Analytics: Assist in analyzing, summarizing, and developing reports on patient safety events and activities across the organization. Monitor action plans and proactive safety assessments of areas with increased risk. Safety Culture Advancement: Support departments in interpreting safety culture survey results and implementing targeted interventions to improve reporting and engagement. Participate in safety culture strategies across the organization. Education, Mentorship & Organization Capacity Building: Educate teams and management on safety science and high reliability best practices. Educate teams and management on incident reporting systems such as reporting cultures, event review best practices, and cause analysis methodology. Serve as a resource for departments on patient safety related issues. Deliver training on various topics related to patient safety and functions within the program. Support Patient Safety Initiatives: Participate in system-wide patient safety improvement projects aligned with organizational goals and high-reliability principles. Lead focused patient safety improvement initiatives. Provide recommendations for improvement and implementation within established workflows and the patient safety program overall. Regulatory & Accreditation: Assist in ensuring compliance with regulatory standards pertaining to patient safety. Support timely reportability determination and documentation that adheres to regulatory requirements. Stakeholder Collaboration Engagement: Facilitate cross-functional collaboration across areas such as Risk, Quality, Nursing, and Operations in areas including but not limited to event review, culture, queries from departments, and policy and/or procedure development. Education Qualifications Bachelor's degree in a work-related discipline/field from an accredited college or university required Master’s degree in a work-related field from an accredited college or university preferred. Experience Qualifications Three (3) years of progressively responsible and directly related work experience Required Knowledge, Skills and Abilities Ability to collect and record data, evaluate data and statistics, and maintain effective reporting systems Ability to communicate effectively, both orally and in writing Ability to foster effective working relationships and build consensus Ability to manage, organize, prioritize, multi -task and adapt to changing priorities Ability to plan, organize, prioritize, work independently and meet deadlines Ability to provide leadership and influence others and effectively consult, advise, and instruct management and staff regarding appropriate approaches for dealing with difficult patient issues. Knowledge of basic research and investigation techniques with an ability to interpret data, prepare reports, and ability to propose solutions to difficult situations involved in a potential or actual patient injury Knowledge of computer systems and software used in functional area – examples include: navigation of electronic health record systems, incident reporting platforms, and Microsoft office tools Knowledge of medical terminology and related levels of care and treatment Knowledge of risk management and quality improvement principles and issues regarding patient safety Knowledge of the current theories, principles, practices, and standards of, as well as emerging technologies, techniques, issues, and approaches in clinical practice Knowledge of the variables that affect the processes and outcomes of patient health care Licenses and Certifications CA-RN (Registered Nurse) CPPS - Certified Professional in Patient Safety preferred CPHQ - Health Care Quality Certification preferred These principles apply to ALL employees: SHC Commitment to Providing an Exceptional Patient & Family Experience Stanford Health Care sets a high standard for delivering value and an exceptional experience for our patients and families. Base Pay Scale: Generally starting at $70.52 - $93.43 per hour The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training. This pay scale is not a promise of a particular wage.
