Physical Therapist Case Manager 🏥 Employer: Signature Albany Home Health 📍 Location: Albany, OR 💲 Pay: $49.00-$55.00 per hour ⏰ Schedule: Full Time Albany Home Health is seeking a compassionate and skilled Physical Therapist to provide in-home rehabilitation services that help patients regain mobility, improve strength, and maintain independence. Join a dedicated team committed to delivering exceptional patient-centered care throughout the Albany community. Key Responsibilities Develop and implement individualized treatment plans. Perform patient assessments, including mobility, gait, strength, and range of motion evaluations. Provide physical therapy services in accordance with physician-directed care plans. Educate patients, families, and caregivers on therapy goals, exercises, and safety measures. Monitor patient progress and maintain accurate clinical documentation. Collaborate with physicians and interdisciplinary healthcare teams. Participate in care conferences and discharge planning. Supervise Physical Therapy Assistants and home health aides as required. Identify additional patient care needs and coordinate appropriate referrals. Qualifications Current Oregon Physical Therapist license. Graduate of an accredited CAPTE-approved Physical Therapy program. Minimum 2 years of Physical Therapist experience; home health experience preferred. Current CPR certification. Excellent communication, organizational, and documentation skills. Valid driver's license, reliable transportation, and current auto insurance.
