Requirements
- Graduate of an accredited Physical Therapy program
- Current Pennsylvania Physical Therapy License
- Minimum two years of physical therapy experience in Home Health or similar settings
- Experience treating geriatric patients in a home health or similar care setting
- Experience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care setting
- Experience providing care in hospice-adjacent contexts
- Valid Driver’s License and reliable vehicle
- Proof of Auto Insurance
- Strong communication skills for effective collaboration with patients, families, and the care team
- Excellent organizational abilities and experience managing a diverse caseload
- Eligible to work in the United States
- Experience with Matrix Care is a plus.
