Job Description: Physical Therapist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase. - Evaluate patients and perform appropriate therapy interventions - Develop a comprehensive plan of care and coordinate multi-disciplinary care - Perform prompt clinical documentation and visit notes - Continued communication with physician, care team members, patient and family - Comfortable working in all aspects of the home health setting - Implement and maintain physical therapy protocols - Manage patient treatment and outcomes directly, or through PTA supervision Requirements: - Current Texas Physical Therapist (PT) license - One year of physical therapy experience - Experience with Homecare Homebase is helpful, but not required - Previous Home Health experience is helpful, but not required - Current Texas driver's license - CPR certification Job Type: - Full-time Schedule: - Monday to Friday License/Certification: - BLS Certification (Required) - Physical Therapy License (Required) EMR Used: - Homecare Homebase Benefits: - Continuing education credits - Dental insurance - 401K - Flexible schedule - Health insurance - Paid time off - Referral program - Travel reimbursement - Vision insurance Work Location: - On the road
