- **Physical Therapist (PT) - Home Health** - **Schedule:** Full-time Monday–Friday - **Location:** Missouri City and its surrounding areas - **Compensation:** $75 per point ### Who We Are: Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health mode. The result: less administrative burden, fewer coordination headaches, and more time for patient care. **Responsibilities:** - **Own the caseload as a case manager:** you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits. - Complete comprehensive in-home assessments and deliver skilled PT visits. - Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention. - Set measurable goals, track objective progress, and adjust treatment quickly when patients stall. - Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving. - Manage treatment and outcomes directly, including **PTA supervision** where appropriate. - Coach patients and caregivers with practical home programs that actually get done. - Document promptly and thoroughly ### Why Adaptive - **Operations that back you up:** You are supported by an experienced clinical and operational team so you can focus on patient care, not chasing logistics. - **Clear productivity model with upside:** You are paid **per productivity point** with an expectation of **28 points/week** - **Schedule flexibility:** We build schedules to make field work sustainable, and we support smart routing and coordination, so your day is predictable whenever possible. - **Cutting-edge tech built for clinicians:** Our charting tools are designed to reduce documentation burden, and our scheduling product will streamline visit coordination and reduce last-minute changes. - **Mission-driven work:** We are here to expand access to high-quality home health care, and we want therapists who take pride in raising the standard. ### Requirements **Must-haves** - Active **Texas PT license** (in good standing) - Strong clinical judgment and comfort working independently in the field - Comfortable escalating changes in condition to RN/MD and documenting rationale clearly - Strong communication and documentation - Reliable follow-through on orders, frequencies, and patient goals - Current driver’s license and comfort with daily travel across assigned territory - Able to carry/transport basic therapy equipment as needed - Competency in core home health PT scope - Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations - Strengthening, functional mobility, and home safety assessment/modification education - Patient/caregiver coaching with practical home programs **Nice-to-haves** - Prior **home health** experience - EMR experience - Deeper clinical experience in high-signal PT populations - Experience supervising/mentoring **PTAs** and collaborating on care plans - Strong organization and time management ### Benefits - 401(k) + 401(k) matching - Health, dental, vision, and life insurance - Paid time off - Flexible scheduling - Referral program - Mileage Reimbursement
