Department: 39122 South Metro F - Home Care Physical Therapy Status: Full time Benefits Eligible: Yes Hou rs Per Week: 40 Schedule Details/Additional Information: Mon- Fri 8-5pm with weekend and holiday rotation in ZIP codes 60620,60628,60643 Pay Range $44.15 - $66.25 Physical Therapist (PT) – Home Health Case Manager Location: Chicago – South Side (servicing ZIP codes 60620, 60628, 60643 and surrounding areas) Schedule: Day shift with rotating weekends and holidays Sign-On Bonus: Eligible for up to $10,000 sign-on bonus for external candidates.
