In this role, you will deliver high-quality, person-centered care in a consultative/co-management capacity to older adults with complex medical and social needs. Highlights: Work closely with primary care clinicians and specialty providers, where you will help guide care plans that promote independence, dignity, and quality of life for aging patients. Practice in a supportive, collaborative environment with access to multidisciplinary team-based resources to ensure coordinated, comprehensive care for every patient. Primary outpatient, with potential for limited practice in skilled nursing facilities/assisted living communities. What You Can Bring: Clinical expertise in geriatric medicine , with a focus on delivering individualized, Age-Friendly care that enhances patient outcomes and quality of life Exceptional communication and interpersonal skills , enabling effective collaboration with patients, families, caregivers, and multidisciplinary care teams A commitment to lifelong learning and professional growth , with enthusiasm for contributing to academic activities such as mentoring trainees, participating in educational programs, and staying current with advances in geriatric care and evidence-based best practices Qualifications: MD or DO degree from an accredited medical school. Board Certification in Geriatric Medicine. Current Pennsylvania medical licensure or eligibility to obtain licensure. Experience in geriatric care is preferred.
