Responsible for data entry for intra-departmental monitoring as well as regulatory monitoring. Develop and format data collection tools as directed.Maintain accurate and confidential records of data collected.Prepares Hospice CAHPS mailing list monthly prior to submission to Press Ganey.Calculate response rates for various surveys and data collection processes. Coordinates the timely and accurate implementation of telephone surveys; collate responses.Enters survey/QA & PI data onto appropriate computer program. Prepares mailings and meeting arrangements and takes minutes for Calvary @ Home QA & I and Professional Advisory Committees and Performance Improvement meetings. Maintains accurate education records on each employee within Calvary @ Home. Collects Medicare D information and notifies appropriate Medicare D Plan upon admission and discharge for Medicare Hospice patients.RequirementsBachelor’s Degree required.Minimum 2 years data entry experience required with accurate typing ability.Knowledge of Microsoft Office (Word, Excel spreadsheets).Knowledge of graphic display of information.
