Top Criteria : • Minimum 3 years clinical experience in the acute care setting. • Proficient with all Microsoft Office computer skills: Outlook calendar, Word, Excel etc. • Telephonic position with 2 late nights until 8pmChronic Condition Mgmt/Disease Mgmt • Education and/or Experience: • Registered Nurse with current Michigan License required • Nursing Diploma or Associates Degree in Nursing • Bachelors degree in Nursing or related fields (preferred) • CCM certification (preferred).
