Progress Monitoring : Participate in the process of monitoring client progress in treatment programs, tracking successes and challenges to inform future treatment plans and strategies. Individualized Treatment Plans : Develop tailored treatment plans for each client, taking into account unique needs, goals, and circumstances to promote optimal recovery and well-being. Client Support and Advocacy : Serve as a trusted resource for clients, providing individualized assistance in accessing necessary mental health services and completing daily living tasks, while advocating for their best interests. Community Engagement : Connect clients with various community agencies and resources, offering ongoing social, vocational, educational, and recreational support and activities to foster personal growth and independence. ***What you’ll bring*** A proactive, and client-focused approach to breaking the stigma of mental health. Associate of Applied Science in Behavioral Health Support degree from an approved institution OR.... Work experience in a related human services field may substitute for educational requirements. *Related fields may include direct support such as case management, residential support, developmentally disabled support, vocational rehabilitation, and more. Four years of qualifying experience minimum or a combination of education and experience equaling four years required. OR....
