Your role is crucial in delivering patient-centered care that supports both patients and their families. **Responsibilities:** - Conduct evaluations and assess home environments to identify speech/language function needs. - Develop and implement plans of care to address functional speech defects. - Administer specialized therapeutic techniques and recommend communication aids as needed. - Instruct patients, families, and caregivers on treatment regimes and collaborate with other healthcare personnel. - Maintain accurate clinical notes and participate in discharge planning using the company's electronic medical record system. - Attend in-services, case conferences, and meetings to ensure coordinated patient care. - Maintain clinical competency in speech-language pathology practices. - May supervise home health aides related to the speech-language therapy plan of care. **Qualifications:** - Master’s or doctoral degree in speech-language pathology (SLP). - Licensed as a speech-language pathologist by the state or fulfilling requirements if the state does not license SLPs. - Minimum of one (1) year's clinical experience in Speech-Language Pathology. - Strong verbal and written communication and organizational skills.
